Jake touched many lives while he was here on earth, in keeping with Jakes passion to help others The Jacob Sloan Foundation has established a secondary fund, our Jacob Sloan Family Assistance Fund. This family assistance fund and hardship grant application is intended to assist families in Queen Anne's County who have recently experienced an emergency or traumatic life event that has caused the need for assistance. Unfortunately, we may not be able to assist all applicants, but we will do our best to review your inquiry and get back to you in a timely manner.
