The Jacob Sloan Family Assistance Fund provides  support to families in Queen Anne's County who have recently experienced an  emergency or traumatic life event that has caused the temporary hardship.

Please make sure to explain what the emergency situation is that has caused your temporary hardship.
Although we may not be able to assist every applicant, we will carefully review each inquiry and respond as soon as possible. Please complete the form below. We will typically contact you by email within 48 hours of receiving your request for more information and any additional documentation we might need to make a determination if we are able to assist. 
If assistance is approved, please allow 7–14 days for processing.
